Making a Referral

Home - Making a Referral
Our referrals mostly come from social workers, medical professionals and lawyers. In some cases, if an estranged family cannot agree on decisions, for example, a relative might seek the appointment of a Welfare Guardian. Applications for Welfare Guardianship must be filed with the Family Court, using special forms. Welfare Guardians Canterbury help our volunteers with the forms and application process. You can also get the necessary forms from the Family Court website or from your local Family Court.
Should you need to refer someone to us for a Welfare Guardian please CLICK HERE to access the Subject Person Referral Form.
